Good Faith Estimate Notice
This notice is a requirement of the federal No Surprises Act:
- You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
- Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
- You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
- For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises.
I am an in-network provider for the Duke University student mental health plan (ACS Benefit Services) only. If you are a Duke student and have the Duke student insurance plan, you simply have a $10 co-payment for psychotherapy a session.
If you have an insurance plan other than this, you likely have Out-of-Network benefits that will cover part of the cost.* If so, I will be glad to file your insurance for you, or supply you monthly with completed forms that have all the information necessary to file your claim yourself. I charge $10.00 per month for filing insurance.
I can also provide you with receipts for a flexible spending health-care account.
*I am NOT a Medicare provider, and so Medicare Advantage plans' out-of-network benefits will not cover my services.
*I am not connected to the NC Healthcare Information Exchange system, so I'm not certain the State Health Plan will still cover my services. If you have SHP insurance, I'll provide statements for your visits so you can file claims, but I can't guarantee they'll provide reimbursement.
Some insurance companies expect the client to pay a portion of their fee at the time of service, and their insurance plan will pay the therapist the balance of the fee; other plans expect the client to pay the therapist their full fee, and will reimburse the client a portion of that fee. Many insurance plans include a deductible that must be met (paid out-of-pocket) before insurance will start to pay. It's important to know your plan's benefits. I include an "Insurance Benefit Information" sheet in new clients' intake packages, which contains a list of question to ask your insurance company.
